		<Abstract>This paper presents the design and implementation of an ambulancebased Health Monitoring System forcontinuous tracking of patient vital parameters during transport The system measures physiological signals suchas heart rate and body temperature using biomedical sensors interfaced with a microcontroller unit The acquireddata is displayed locally on an onboard LCD screen for immediate observation by the caretaker inside theambulanceIn addition the system hosts a dedicated web interface accessible through a unique devicespecific URLenabling realtime monitoring of patient data by hospital staff This facilitates early preparation for medicalintervention prior to the patients arrival A thresholdbased alert mechanism is incorporated wherein abnormalvital readings trigger a buzzer within the ambulance to notify the caretaker Furthermore a Telegrambasednotification system is implemented to provide continuous remote alerts to authorized personnel when abnormalconditions are detectedThe proposed system offers a practical and costeffective solution for prehospital patient monitoring enhancingcommunication between ambulance staff and healthcare providers The implementation demonstrates reliablerealtime data transmission under standard network conditions while limitations include dependency onnetwork availability and sensor accuracy</Abstract>
